| $250.00 base rate for the first firm only. There will be no base fee for additional firms with the same ownership. All firms under the same ownership must be members if one firm is to be a member. |

$6.00 per case for the first 300 cases.
$3.00 per case for the next 301-600 cases.
$1.50 per case for cases exceeding 600.

NOTE: The sliding scale of PAC contribution is included in the dues amount. $3 per case will go towards PAC. If you choose not to contribute to the PAC, the amount will be deposited in the general NYSFDA account. Please indicate on the "membership application" by checking in the appropriate box - ONLY if you choose not to invest in PAC.
Payment Options
Applicants may pay using either check or credit card (Visa/MC) in the space provided on the membership application or they may choose to have a monthly installment taken out of your checking account.
